Mental health checklist

The professionals at Grosse Pointe Manor Nursing and Rehabilitation Center recognize that illness, injury, and age can affect mental health. We know it is difficult to face these problems, but acknowleding a mental health condition is the first step to improving your loved one's care and comfort. Write up an assessment of your loved one's mental health, considering the following points. Note that a primary care doctor can add insight, but consultation with a geriatric psychiatrist or neurologist might identify more issues so that the best care can be provided.

  • Diagnosis of any psychiatric disorders such as depression, anxiety, or psychosis
  • Diagnosis of Alzheimer’s or other form of dementia
  • Recent hospitalizations for any of these problems
  • Mood swings, rage, or hostility
  • Forgetfulness or wandering off
  • Sadness or loneliness
  • Decreased interest in reading, writing and communicating
  • Difficulty in maintaining friends
  • Decreased interest in life
